Dr. Corey Jensen completed his first two years of training in comprehensive orthodontic care in the late 1980’s. At that time we were limited to traditional braces and some appliance therapy. Since then, with the development of aligner therapy – more commonly known as Invisalign, the world of orthodontics has changed forever. Up until about 10 years ago, 90% of the orthodontic care we provided was performed on children and adolescents. Today, however, because of the ease and speed of care using Invisalign we are treating as many adults as we are kids. We routinely hear from our patients, “I did not expect how fast, how easy or how affordable the Invisalign treatment was.”

Straightened teethOur doctors and team believe that there is much more to a beautiful smile than just straight teeth. As we create our orthodontic plan and smile design for our patients we consider many factors beyond alignment. Equally important to having a beautiful smile is having a well-functioning bite. Teeth that are positioned correctly can withstand heavier biting forces, are less prone to breaking or cracking and will wear less over time.

Our basic goal is to have all the back teeth touching evenly when the teeth are biting and when the teeth are sliding front to back or side to side they are being guided by the front teeth only. By creating this front teeth guiding pattern the back teeth are then being protected from the heavy forces that our strong jaw muscles can produce.

Other factors that we may discuss enhancing might include; teeth color, gumline contours and heights, teeth shapes, asymmetries, worn or chipped edges and lip positions. All of these must be considered when as part of the ultimate smile design.

Our orthodontic process:

Step 1: A free orthodontic consultation:

At Smile Design we begin all of our orthodontic treatment, whether we do braces or Invisalign, with a free consultation. At that appointment we evaluate many factors and can begin outlining a list of dental problems and potential solutions. We begin by discussing problems we may find and each of the possible treatment options, estimating approximate treatment duration and approximate costs of treatment.

Step 2: Orthodontic records [Our information gathering appointment]

For families that are interested in proceeding with their orthodontic care we ask them to return for an information gathering appointment. We refer to this as ‘Orthodontic records’. This consists of a panoramic x-ray, a cephalometric x-ray, a series of photographs and impressions to make study models. See below:

From these orthodontic records our doctors will create a comprehensive treatment plan to better estimate treatment duration, treatment options and final costs.

Step 3: Our final orthodontic consultation:

We then ask the patient or family to return for a final discussion prior to beginning our treatment. Once a plan has been agreed upon we are ready to begin.

Step 4: Beginning treatment:

Traditional orthodontic braces:

For our patients electing traditional braces we then begin with about a 90-minute appointment to bond the brackets, place an archwire and attach the colored rubber band to each bracket. Choosing colors seems to always be one of the highlights for our patients. Once we have completed placing the braces we then have a short discussion about oral hygiene, diet, and precautions to avoid damaging the braces.

Following the initial placement of the braces we generally have our patients return every 4-8 weeks for checks and necessary adjustments. Once we have achieved the final result we are then able to remove the brackets and make retainers to maintain the beautiful smile we have created. The clear retainers that we use are worn full time for the first six months and then worn while sleeping only from that point on.

Invisalign orthodontic treatment:

For our patients choosing to forgo braces on their teeth and instead use aligners we begin the Invisalign process. Once we have completed our final consultation and have our plan in place we use our I-Tero Scanner to do a 3D scan of all the teeth.

We then digitally send this to the Invisalign Company where they create a virtual demonstration of the teeth going through the alignment process. This plan is known as a “Clincheck”. Generally, the doctor will receive this program from Invisalign within a week of submitting it. Our doctors then have the opportunity to make desired changes before accepting the plan and putting it into production. Generally from the day we do our final scan we can begin treatment within a few weeks. The first appointment is a quick and easy one. We deliver the first set of aligners, give wear instructions of 22 hours per day and review hygiene and diet. One of the most exciting breakthroughs with Invisalign recently occurred in the fall of 2016 where aligners are now worn only one week each rather than two weeks which had always been the standard protocol for treatment.

20 years ago, orthodontic treatment was rarely completed in less than 2 years and often was described as being very uncomfortable. Today with our highly engineered brackets, space age nickel-titanium wires and aligner treatment options we often see the same types of treatments being completed in about a year with minimal reports of pain or discomfort.

Generally we then see our Invisalign patients every 4-8 weeks. They are given enough aligners to make the weekly changes between appointments. At the completion of active movement we ask our patients to continue to wear retainers full time for the first 6 months and then while sleeping from that point on.
